We provide direct support procured by Assessment and Care Management (ACM) this includes funding from continuing health care and social care.
We are directly funded through individual’s support plans and direct payments due to the complexity and availability of the people we support and their diverse range of needs. Our customer base has grown through individual social work recommendation, family recommendation and reputation due to delivering a quality support service.
We and all our support workers work with the Reach Standards:
I choose who I live with, I choose where I live, I choose who supports me, I choose how I am supported, I choose what happens in my own home, I have my own home, I make friendships and relationships with people on my terms, I am supported to be healthy and safe on my terms, I have the same rights and responsibilities as other citizens.
We support our clients with many various health and well-being activities such as supporting individuals out in the community to fitness sessions, gyms, swimming, accessible football sessions and accessing GP healthy living workshops within their own communities.
We assist individuals to any health appointments, on-going GP visits, annual learning disability health checks, sexual health appointment’s and well woman clinic, we work in a multi-disciplinary way to ensure our clients general health, mental health and general well-being is maintained to reduce the risk of ill health.
As part of our service we offer support with cooking and healthy eating plans. Support Workers assist individuals in areas such as menu planning, food shopping, preparing healthy meals, researching recipes, block cooking food and freezing to ensure healthy meals are available out of support hours and supporting individuals to keep a food diary to monitor a steady balanced diet, to reduce the risk of weight gain/ weight loss which may lead to ill health.
We encourage natural friendships and relationship circles to reduce paid support in people’s lives and promote social networking to reduce individuals feeling lonely, isolated, depressed and vulnerable. For example we support individuals to plan activities with others to access the community events such as Under The Stars, theatre evenings, cinema outings, BBQ’s, day trips etc using shared support or no support for some people to reduce costs also promoting peer support.
We also seek volunteer placements and community workshops for individuals to access reduce paid support but also to enhance individual’s day time opportunities.
